Open means recruiting, not yet recruiting, enrolling by invitation or active and no longer recruiting, with at least one US site and a drug or biologic intervention. A company is counted once, with all of its open studies. These 6,000 studies are the US share of each company's work, not its whole pipeline. A combined phase 2 and 3 study counts as phase 3. Rounded figures may not add to the total.

Groups this page cannot size, said in words rather than shown as a number.

The size of a company's medical affairs, portfolio or communications team is not published anywhere we can count. Companies selling approved drugs with no open US trial sit outside the trial counts, though any US sites they run in these industries sit inside the location counts. Programs still before their first trial are the same. Each of these is built by name, company by company.

About 160 companies lead five or more open US studies, and about 30 of them lead 20 or more. Your site describes portfolio leadership for organizations with no consolidated view across 50+ R&D or transformation projects. Of the groups on this page, these companies come closest to that description, the largest of them most of all. The approach goes to the head of portfolio management or R&D operations.

Companies with a phase 3 open and no phase 4.

About 540 companies have an open US phase 3 trial and no open phase 4 study. Late-stage programs are often where medical affairs and KOL work builds ahead of a launch, and your site lists newsletters for medical affairs teams and a module for KOL and HCP engagement. The approach goes to the head of medical affairs.

Companies leading one open study.

About 1,100 companies lead a single open US study. Many are small developers, and at those the board deck, the pipeline update and internal news often sit with the chief executive and a small leadership team, so one conversation can cover what your site sells as several solutions.

Denominators, definitions and sources

  1. Trial counts come from the federal trial registry, read on Oct 7, 2026. They count interventional studies with an intervention of the registry's drug or biological type, that are open and name at least one US site, and whose lead sponsor is industry. Company names are matched after removing legal suffixes, so a company listed under two spellings, or a parent and its subsidiary, may count twice.
  2. Some companies have neither an early-phase-only nor a phase 3 profile (for example only phase 4 studies, or only studies with no phase given), so those two counts do not add up to the company count.
  3. Location counts are establishments in the federal annual count of business establishments by industry, 2023, national file, all legal forms. An establishment is one physical location with paid employees. Staff is paid employees at the location in the mid-March 2023 pay period. Where the file withholds a size cell, the withheld locations are recovered as the group total less the published cells.
  4. Industry codes 325412 pharmaceutical preparation manufacturing; 325414 biological product manufacturing, except diagnostic; 541714 research and development in biotechnology, except nanobiotechnology.
  5. Built from public registries, counts banded deliberately. Figures of 100 and over are rounded to two significant figures, figures from 10 to 99 to the nearest ten.
